Now that you have booked or requested your first appointment with us we need to know a little more about your medical and dental history before you see the dentist. We request that new patients arrive 15 minutes early in order to complete a medical history form, alternatively, you can print off and complete our Confidential Medical and Dental Questionnaire, a copy of which is available to download below.”
